Inside Warehouse and Unloading 18 Wheeled Trucks: Yale Lift Truck Model ERC040VAN36TE082: Serial Number A969N01564J: DECA Property DE0094952: Employees were operating forklift to move pallets loaded with supplies. Employer had not retained annual maintenance record for the fire extinguisher located on forklift. Fire extinguisher had mechanical damage to the bottom. Employees were exposed to struck-by and burn hazards.

Employer had not notified DECA Manager that fire extinguisher had mechanical damage and needed to have a new hydrostatic test conducted. Employer did not remove the fire extinguisher from service. Employees were exposed to struck-by and burn hazards.
