Employee was observed cutting wood materials using a circular saw without eye/face protection.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: At the residential construction site located at 1215 Shoal Creek Parkway in Kansas City, Missouri: Employees engaged in construction activities were exposed serious fall injuries, in that a JCB 532 Loadall telehandler was being used to support an elevated makeshift work platform. The platform was a flat platform that was not specifically designed and approved by the manufacturer for this task.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: At the residential construction site located at 1215 Shoal Creek Parkway in Kansas City, Missouri: Employees engaged in construction activities were exposed serious fall injuries. An employee working from a platform approximately 12 feet above the ground, supported by a JCB 532 Loadall telehandler. The scaffold platform did not have guardrails and the employee had no fall protection.
