OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to caught-in hazards.    The employer has not developed specific LOTO procedures for employees required to perform maintenance on machinery located throughout the facility at the Silvercot Gin.

29 CFR 1928.57(a)(7)(i): The employer did not protect employees from coming into contact with hazards created by moving machinery parts through the installation and use of a guard or shield or guarding by location:      On or about December 4, 2019, employees working at the Silver Cot Gin were exposed to unguarded pulleys when employees open the door to unchoke the #32 Rescuer 3000 stick machine.
