Location: The Roof of 725 Main Street Millis, MA On or about December 6, 2019 employees were exposed to struck by hazards from falling constriction debris, where hard hats were not worn.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Location: The Roof of 725 Main Street Millis, MA On or about December 6, 2019 employees were exposed to fall hazards of more than 20 feet from a roof line to the ground below, where fall protection was not in use.
