On or about December 9th, 2019, at 500 N Broadwalk, Hollywood, FL 33019, employees were exposed to a 11-foot fall hazard while in the process of performing stucco work on a roof of a commercial structure and not using any means of a fall protection system.

On or about December 9th 2019, at 500 N Broadwalk, Hollywood, FL 33019, employees were exposed to an 11 feet fall hazard when using an extension ladder which did not extend at least 3 feet above the upper landing surface and was not secured from being displaced while gaining access to the lower roof .
