29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:  a) Shop area, horizontal saw; On or about December 3, 2019 and at times prior to, the employer required that employees conduct maintenance activities such as changing out saw blades on the horizontal saw without providing training on lock out/tag out methods and means for energy isolation.  b) Shop area, angle saw; On or about December 3, 2019 and at times prior to, the employer required that employees conduct maintenance activities such as changing out saw blades on the angle saw without providing training on lock out/tag out methods and means for energy isolation.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment:  a) Saw area, horizontal saw; On or about December 3, 2019 and at times prior to, the employer allowed employees to conduct maintenance on a saw without providing employees with the means to affectively apply locks for each employee involved in the maintenance of that machine.  b) Saw area, angle saw; On or about December 6, 2019 and at times prior to, the employer allowed employees to conduct maintenance on a saw without providing employees with the means to affectively apply locks for each employee involved in the maintenance of that machine.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Press Break Machine; On or about December 3, 2019 and at times prior to, the employer required the use of two employees to operate a press break machine to bend mild steel without providing each operator involved with a method of positive control or the operation exposing the employees to amputation hazards.  b) Drill press; On or about December 3, 2019 and at times prior to, the employer required the use of a drill press machine without providing protection to employees from the point of operation exposing the employees to puncture hazards.
