29 CFR 1926.651(i)(3): Sidewalks, pavements, or appurtenant structures had been undermined and a support system or another method was not provided to protect employees from the possible collapse of such structures.    a.  On or about December 10, 2019, at the above addressed jobsite, employees conducting diesel pump installation operations in a trench/ excavation undermined the pavement by three (3') feet four (4") inches, thereby exposing employees to the hazards associated with a pavement collapse.

On or about December 10, 2019, at the above addressed jobsite, an inspection of the trench/ excavation was not performed by a competent person, thereby exposing employees to the hazards associated with cave-ins.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).    a.  On or about December 10, 2019, at the above addressed jobsite, employees were conducting diesel pump installation operations in a trench/ excavation measured at six (6) feet in depth.  The trench was not provided with cave in protection, thereby exposing the employees to cave in hazards.
