29 CFR 1910.95(k)(1): The employer did not train each employee who is exposed to noise at or above an 8-hour time-weighted average of 85 decibels in accordance with the requirements of 29 CFR 1910.95(k):   On or about January 30th, 2020, at this establishment, where employees operated and manufactured different lines of the plastic film and bags products, were exposed to noise hazards without an initial effective hearing conservation training program in place:  The Plastic Box Machine #16 Operator/Packer was exposed to a noise level of 93.20 dBA during a 418 minutes sampling period.

29 CFR 1910.95(k)(2): The training program was not repeated annually for each employee included in the hearing conservation program:   On or about January 30th, 2020, at this establishment, where employees operated and manufactured different lines of the plastic film and bags products, were exposed to noise hazards without a continuing and effective annually hearing conservation training program in place:  a. The Plastic Extrusions Line #19 Operator was exposed to a noise level of 88.10 dBA during a 436 minutes sampling period; b. The Plastic Bags Automation Machine Technician was exposed to a noise level of 89.20 dBA during a 432 minutes sampling period; c. The Plastic Box Machine #19 Operator/Packer was exposed to a noise level of 88.90 dBA during a 423 minutes sampling period; and, d. The Plastic Box Machine #16 Operator/Packer was exposed to a noise level of 93.20 dBA during a 418 minutes sampling period.

The Plastic Extrusions Line #19 Operator was exposed to a noise level of 88.10 dBA during a 436 minutes sampling period; b. 29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed:   At this establishment, employees were exposed to struck-by, caught-in or -between, amputation and electrical hazards when performing repairs, service and/or maintenance of energized machinery and equipment. Inspections of the energy control procedures were not conducted at least annually for all machines and equipment such as, but not limited to:  a. Case Palletizing Robot Machine, 11-E-6; b. Plastic Bags Automation Robot, Fanuc M710 iG; c. Plastic Film Extruder Line Machine #11 to #19, Kiefel CA6; d. Plastic Bags Automation Machine, Genesis #2372; e. Plastic Box Machine #16 and #19, ADCO 15DZ-60-SS; f. Drawstring Plastic Line Bag Machine, CMD 1270GDS; and, g. Drawstring Plastic Line Winding Machine, CMD 4213RO.
