29 CFR 1926.502(d)(17): Personal fall arrest systems. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. Costal Campus Silver Strand Training Complex New Build, Boiler Room Rooftop: Employee(s) were installing waterproof membrane along the edge of the flat roof. Employees were using full body harnesses with the straps not adjusted properly and the D-rings were located on their lower backs. Employees were exposed to fall hazard(s) of 14.4 feet to the lower level.
