29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled: (a) Yard area; On or about December 6, 2019, a forklift spreader bar used to lift a bundle of metal sheets was not securely attached to the forklift forks. A delivery driver was hospitalized after being struck by a spreader bar (forklift attachment) and/or load that came off the forklift forks.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: (a) Yard area; A Komatsu brand forklift was not equipped with markings for each attachment used on it, such as a custom made spreader bar.
