29 CFR 1910.110(f)(6)(i)(a): Storage outside of buildings, for containers awaiting use or resale, shall be located in accordance with Table H-33 with respect to: The nearest important building or group of buildings The amount of LP-Gas containers stored inside the permanently docked semi-tractor trailers, 12 metal cages, 12 containers, 33.5 lbs. of propane in each container, in each cage, a total quantity of 4,824 lbs. in each tractor trailer, abutting Dock Doors #48 and 302 where loading and unloading of freight on a busy thoroughfare on each side of the propane storage dock door and were not stored at least 10 feet away from the building.

29 CFR 1910.176(a): Use of mechanical equipment. Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways shall be appropriately marked. On December 5, 2019, in the aisle way at Dock door #48, a forklift operator parked perpendicular in the aisle way to change a propane tank and created an obstruction in the 15 ft. aisle way and a struck-by hazard from another forklift operating in the area.

29 CFR 1910.178(l)(3)(ii)(I): Powered Industrial Trucks: Other unique or potentially hazardous environmental conditions in the workplace that could affect safe operation. The employer's training program for powered industrial trucks did not address where, during the changing of the propane tank, truck operators are to park their trucks to avoid being struck by other powered industrial trucks operating in the aisle ways near Dock Doors 48 and 302.

29 CFR 1910.178(n)(6): The driver shall be required to look in the direction of, and keep a clear view of the path of travel. On December 5, 2019, around Dock Door #48, a forklift operator was changing a propane tank and another forklift operator was backing up and did not look in the direction of travel and struck the forklift operator.
