29 CFR 1910.212(a)(3)(iv)[e]: The point of operation on milling machines whose operation exposes an employee to injury shall be guarded: (a) Intermountain Electronics at 8221 E 96th Ave, Unit A, Henderson, CO 80640: On or before December 12, 2019 the employer did not ensure that guarding was provided to protect operators and other employees from the hazards created by the rotating parts of a Bridgeport milling machine. An employee operating the equipment sustained amputation injuries after the rag he had been holding contacted the spinning bit, thus wrapping the rag and the employees hand around the bit assembly. This condition exposed employees to amputation and laceration hazards.
