29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part:  Employees were exposed to struck-by and fall hazards.  At the jobsite located at 1404 N. Pinecrest Wichita, KS:  Employees engaged in roofing activities were exposed to struck-by and fall hazards while using pitch forks and working from heights greater than 6 feet above a lower level.  Employees were exposed in that the employer had not developed and maintained a comprehensive safety and health program which addressed these and other hazards.

At the jobsite located at 1404 N. Pinecrest Wichita, KS:  Employees engaged in commercial roofing activities, walking and working on a low-slope roof, were exposed to fall hazards of approximately 9 feet 5 inches from the eave of the building to the ground below in that fall protection was not utilized.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  Employees were exposed to fall hazards.  At the jobsite located at 1404 N. Pinecrest Wichita, KS:  Employees engaged in roofing activities using a portable ladder were exposed to fall hazards of approximately 9 feet 5 inches from the eave of the building to the ground below. Employees were exposed in that the ladder was not secured to the building and did not extend at least 3 feet above the eave.
