29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:   A)     Federal Contractor, LLC, conducting work at 107 Beaver Creek Trail, Amherst, OH 44001: On or about December 12, 2019, employees were performing residential roofing activities without the benefit of an effective accident prevention program to ensure employees were protected from hazards that can result in eye injuries through the use of eye protection, and from fall hazards of approximately 8 feet to the ground below through proper utilization and use of the fall protection and ladders.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   A)     Federal Contractor, LLC, conducting work at 107 Beaver Creek Trail, Amherst, OH 44001: On or about December 12, 2019, the employer did not ensure that employees were adequately trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems, or alternative measures of fall protection while conducting roofing activities atop the one story residential structure. Employees were exposed to approximately 8 feet fall hazard from the roof of the structure to the ground below.

Employees were exposed to the hazards that could result in eye injuries.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   A)     Federal Contractor, LLC, conducting work at 107 Beaver Creek Trail, Amherst, OH 44001: On or about December 12, 2019, the employer did not ensure that the extension ladder, used to access the roof of the one story residential structure, was extended at least three feet above the landing surface. Employees were exposed to approximately 8 feet fall hazard when using the ladders to access the roof in order to conduct construction activities.
