29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. a)On 12/12/2019 the employer did not obtain written approval from the manufacturer for the process of rigging an I-beam to one fork of a Toyota Forklift (Model # 5FGC25), known as free rigging. During the process an employee was assisting a forklift operator to place the 20 ft., 980 lbs. I-beam onto a set of rollers. The forklift operator had set the end of the I-beam on a roller and was in the process of lower the other end of the I-beam onto the rollers when the employee moved to the middle section to guide the I-beam down to the rollers, while the I-beam was being lowered onto the rollers the employee's right hand was caught between the fork and the I-beam.

29 CFR 1910.178(m)(2): No person shall be allowed to stand or pass under the elevated portion of any truck, whether loaded or empty. a) On 12/4/2019 an employee was assisting a forklift operator (Toyota Model # 5FGC25) to place a 20 ft., 980 lbs. I-beam onto a set of rollers. The forklift operator had set the end of the I-beam on a roller and was in the process of lower the other end of the I-beam onto the rollers when the employee moved to the middle section to guide the I-beam, in this process the employee's right hand was caught between the fork and the I-beam.

Caution shall be exercised when handling off-center loads which cannot be centered. a) On 12/12/2019 the employer did not ensure that loads were stable while employees were rigging an I-beam (20ft., 980 lbs.)
