29 CFR 1915.89(g)(1): Before servicing machinery, equipment, or a system that has a lockout/tags-plus system, the employer shall ensure that the authorized employee, or the primary authorized employee in a group lockout/tags-plus application, verifies that the machinery, equipment, or system is deenergized and all energy sources isolated. a) The Electrical Gallery at Dry Dock #5: Employer did not ensure that two employees working on the electrical panel #F-12 verified that the machinery, equipment, or system was deenergized and all energy sources isolated.
