29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support):  JOBSITE: The Werner 28-foot aluminum extension ladder used by employees to access the roof was set at an angle such that the horizontal distance from the top support to the foot of the ladder was greater than one fourth (1/4) the working length of the ladder (the distance along the ladder between the foot and the top support).

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, unless another provision in paragraph (b) of this section provided for an alternative fall protection measure:  JOBSITE: The employer did not ensure that the employees performing roofing work on a steep (6 in 12 pitch) roof were protected from the hazard of falling approximately 12 feet 7 inches to 18 feet 4 inches to the ground by the use of guardrails, personal fall arrest systems and/or other equivalent means.  Valderramos Construction, LLC was previously cited for a violation of this Occupational Safety and Health standard {29 CFR 1926.501(b)(13)}, which was contained in OSHA Inspection Number 1320984, Citation Number 01, Item Number 002a, and was affirmed as a final order on 08/22/2018, with respect to a workplace located at 151 Westover Road, Stamford, CT 06902.
