29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: On or about December 12, 2019 and times prior thereto, the employer used a DBI Sala forklift cage attachment for the Crown RC forklift without manufacturer's approval, exposing employees to the hazard of fall.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: On or about December 12, 20019 and times prior thereto, the electrical control panels near the front office powering lights and other items to the facility was blocked by pallets, exposing employees to the hazard of electrical shock.
