This violation was observed on December 11, 2019, when employees working on lower levels of scaffold were exposed to struck-by hazards from materials and/or hand tools from upper level scaffold platforms. 29 CFR 1926.451(e)(1): The employer did not ensure that when scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was used. Cross braces shall not be used as a means of access. This violation was observed on December 11, 2019, when employees working on a single story commercial building were exposed to falls of more than 12 feet to the concrete below. No ladder or other safe means of access was provided.
