29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:   a) On or about December 13, 2019, at the manufacturing facility, the employer exposed employees to respiratory hazards, in that, a program was not implemented for employees wearing a 3 M half-face elastomeric respirator while performing tasks such as, but not limited to metal grinding.  The Sundog Structures LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(c)(1), which was contained in OSHA inspection number 1394875, citation number 02, item number 01 and was affirmed as a final order on 10/24/2019, with respect to a workplace located at 4909 W Knollwood Street Tampa, FL  33634.

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:   a) On or about December 13, 2019, at the manufacturing facility, the employer exposed employees to respiratory hazards, in that, a medical evaluation was not completed for employees who were provided and wore a 3 M half-face elastomeric respirator while performing tasks such as, but not limited to metal grinding.  The Sundog Structures LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1910.134(e)(1), which was contained in OSHA inspection number 1394875, citation number 02, item number 02 and was affirmed as a final order on 10/24/2019, with respect to a workplace located at 4909 Knowllwood Street Tampa, FL  33634.

29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator and other employees were not fit-tested annually:  a) On or about December 13, 2019, at the manufacturing facility, the employer exposed employees to respiratory hazards, in that, a fit testing was not completed for employees who were provided and wore a 3 M half-face elastomeric respirator while performing tasks such as, but not limited to metal grinding.  The Sundog Structures LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(f)(2), which was contained in OSHA inspection number 1394875, citation 2, item number 3 and was affirmed as a final order on 10/24/2019, with respect to a workplace located at 4909 W Knollwood Street Tampa, FL  33634.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):   a)  On or about December 13, 2019, at the manufacturing facility, the employer exposed employees to injuries, in that, an evaluation of authorized operators for Unicarriers Forklift, Model FHG35SL-A1 used to unload and move shipping containers had not been completed.  Sundog Structures LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard [29 CFR 1910.178(l)(1)(i)], which was contained in OSHA inspection number 1394875, citation number 02, item number 01 and was affirmed as a final order on 10/24/2019, with respect to a workplace located at 4909 W. (a)  On or about December 13, 2019, in the manufacturing area, for employees conducting metal grinding and sanding around chemicals such as but not limited to Florida Paints-Ironman 5450 Alkyd Metal Primer, PPG Pitt-Tech Satin White/Pastel Base and Klean Strip Fuel Denatured Alcohol.  The Sundog Structures LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1200 (e)(1), which was contained in OSHA inspection number 1394875, citation number 2, item number 7 and was affirmed as a final order on 10/24/2019, with respect to a workplace located at 4909 W Knollwood Street Tampa FL  33634.
