29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a- 3049 Fernwood Avenue, Egg Harbor Township NJ: Workers, not using fall protection, were exposed to fall hazards in excess of 14 feet to the ground surface below when performing roof shingle rip, tear, replace and other related activities at the worksite.  On or about 12/12/19.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.1053(a)(1): Ladders shall be capable of supporting the following loads without failure:  a- 3049 Fernwood Avenue, Egg Harbor Township NJ: On or about 12/12/19 a worker fell, in excess of 6 feet to the ground surface below, while using an extension ladder whose fly section was not locked into position and thus not capable of supporting a load without failure.     NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    a- 3049 Fernwood Avenue, Egg Harbor Township NJ: On or about 12/12/19 two workers fell, in excess of 6 feet to the ground surface below, while using an extension ladder that was not secured at its top.     NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. 29 CFR 1926.1053(b)(7): Ladder(s) that were unsecured or not provided with slip-resistant feet were used on slippery surfaces:    a- 3049 Fernwood Avenue, Egg Harbor Township NJ: On or about 12/12/19 company workers were exposed to fall hazards,  in excess of 14 feet to the ground surface below, while using an unsecured extension ladder whose feet were set on a plastic tarp.     NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
