On or about December 16, 2019 at the construction site, employees were exposed to struck-by hazard while operating nail guns without wearing eye protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about December 16, 2019 at the front area of the construction site, employees were exposed to fall hazard when using an extension ladder to access a roof without extending the ladder side rails at least 3 feet above the roof landing.
