29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  (a) In the production floor of the Kodiak Gypsum Mountain States, LLC facility where the employer had not provided two production employees, who had been given dust masks to wear on a voluntary basis, the information in Appendix D of 29 CFR 1910.134 which provided information to employees so that certain precautions were taken to ensure that the respirator itself did not present a hazard.  Kodiak Gypsum Mountain States, LLC was previously cited for violation of this occupational safety and health standard or its equivalent standard 1910.134(c)(2)(i), which was contained in OSHA Inspection 1310527, Citation 2 Item 1 and was affirmed as a final order on June 20, 2018.
