29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:   A.  On or about 12/15/2019, 740 Route 30, Vernon, CT job site South side:  Employees were exposed to electrical shock hazards.  Employees used an aluminum extension ladder, approximately 4 feet away from energized power lines.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    A.   On or about 12/16/19, 740 Route 30, Vernon, CT job site:  Employees engaged in residential roofing activities on a steep roof, 8 in 12 pitch,  employees were exposed to falls of approximately 8 feet to the ground below. Employees were not protected by guardrail systems, safety net system, or personal fall arrest system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  A.  On or about 12/16/19,  740 Route 30, Vernon, CT job site:   1.   West side of house:  Employees used an aluminum extension ladder, that was not fully extended, to access the roof. The ladder side rails were not extended at least 3 feet above the roof landing surface.   2.    Back of house:  Employees used an articulating Little Giant ladder to access the roof.  The ladder was approximately one foot below the roof surface.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways:  A.   On or about 12/16/19, 740 Route 30, Vernon, CT job site: Three (3) employees used an aluminum extension ladder and an articulating ladder that did not reach roof landing surface plus three (3) feet past roof landing surface.  The employer had not provided employees with training on the safe use of ladders employees were exposed to fall hazards.
