29 CFR 1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel.     In the welding shop, the employer did not ensure adjustable tongue guards were installed prior to operating the table grinder. Employees were exposed to stuck-by hazards.

29 CFR 1910.305(g)(2)(ii): Hard-service cord and junior hard-service cord No. 14 and larger were repaired without a spliced:Note:  Hard service flexible cords No. 12 or larger may be spliced if the splice retains the insulation, outer sheath properties, and usage characteristics of the cord being spliced.      In the welding shop area, the employer had in service a portable welding machine with the power cord spliced exposing employees electrical shock hazards.
