29 CFR 1926.502(d)(11): Lifelines were not protected against being cut or abraded:  a) Schwartz's Siding Roofing & Renovations LLC located at 230 E. Long Street, Columbus, OH 43215:  On and preceding December 10, 2019, the controlling employer did not ensure that vertical lifelines used for fall protection at or near leading edges were protected against being cut.  A vertical lifeline, rope grab was used as a personal fall arrest system (PFAS) by the framing crews near 90 degree leading edges while installing parapet walls and wood decking at heights beyond 6 feet.  The lifeline was not protected from being cut by the 90 degree edge in the event of a fall.  The following condition exposed the sub-contractor employees to falls in excess of 70 feet.

Long Street, Columbus, OH 43215:  On and preceding December 10, 2019, the controlling employer did not ensure that vertical lifelines used for fall protection were rigged so that employees could not free fall more than 6 feet nor contact any lower level.  A vertical lifeline, rope grab was used as a personal fall arrest system (PFAS) by the framing crews near 90 degree leading edges while installing parapet walls and wood decking at heights beyond 6 feet.  Lifelines were not rigged in a way to limit fall distance to 6 feet and or keep the worker from contacting the lower level.  The following condition exposed sub-contractor employees to falls exceeding 70 feet.
