29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a)  At the jobsite, on the roof adjacent to the project building, on or about 12/13/19: Employees were building a knee-wall near the edge of a 10 foot fall hazard without fall protection of any kind.    b)  At the jobsite, on the deck at the rear of the project building, on or about 12/13/19:  An employee was working on a partially covered working surface exposed to a fall of 10 feet without fall protection of any kind.    c)  At the jobsite, on the second floor inside the project building, on or about 12/13/19:  Employees were exposed to 10 foot fall hazards where guard rails-were removed from 2 stairwells.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around the opening:  a)  At the jobsite, on the deck at the rear of the project building, on or about 12/13/19: Employees were working and transiting across the unfinished surface, where they were exposed to gaps in the decking.    b)  At the jobsite, on the second floor of the project building, on or about 12/13/19:  Employees were transiting an area where the opening for a utility chase exposed workers to injury.
