OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees exposed to a struck by hazard from the below the hook attachment. a) On or about 12/18/2019 the employer did not ensure that the vacuum sensors on the Anver Electric Powered Vacuum Generator lifting attachments (model # VPF-57-AC) & (model # VSL-08) were working properly exposing employees to a struck by hazard. b) On or about 12/18/2019 the employer did not ensure that a periodic inspection was conducted on below the hook crane attachments such as but not limited to: the Anver Electric Powered Vacuum Generator lifting attachments (model # VPF-57-AC) & (model # VSL-08).

Test locations as follows: Test location #1 was at the Plasma Cutter, it measured at 60 PSI.
