OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees exposed to a struck by hazard from the below the hook attachment.  a) On or about 12/18/2019 the employer did not ensure that the vacuum sensors on the Anver Electric Powered Vacuum Generator lifting attachments (model # VPF-57-AC) & (model # VSL-08) were working properly exposing employees to a struck by hazard.    b) On or about 12/18/2019 the employer did not ensure that a periodic inspection was conducted on below the hook crane attachments such as but not limited to:  the Anver Electric Powered Vacuum Generator lifting attachments (model # VPF-57-AC) & (model # VSL-08).

29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition shall be removed from service. All repairs shall be made by authorized personnel.    a)On or about 12/18/2019 the employer did not ensure that powered industrial trucks (2 Hyster Forklifts (Model # BB0XLBC8) and (Model # H80CXL))with known unsafe condition were taken out of service.   Mechanical issues such as but not limited to: hydraulic leaks and back-up light were not working.

Alloy steel chain slings shall have permanently affixed durable identification stating size, grade, rated capacity, and reach.    a) On or about 12/18/2019 the employer did not ensure that the alloy steel chain slings had a readable permanently affixed tag.  Employees were using chain slings with no rated capacity to offload metal parts.

Compressed air shall not be used for cleaning purposes except where reduced to less than 30 p.s.i. and then only with effective chip guarding and personal protective equipment.    a) On or about 12/18/2019 the employer did not properly reduce the PSI for the compressed air utilized by employees for the cleaning of themselves or equipment exposing employees to hazards associated with compressed air measured at greater than 30 PS1.    Test locations as follows:  Test location #1 was at the Plasma Cutter, it measured at 60 PSI.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage shall be separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour.    a) On or about 12/18/2019 the employer did not ensure that oxygen cylinders were separated from fuel-gas cylinders such as but not limited to: acetylene cylinders exposing employees to struck by hazards.    b) On or about 12/18/2019 the employer did not ensure that compressed gas cylinders were secured from being knocked over exposing employees to struck by hazards.
