1910.147(d)(4)(ii): Lockout devices were not affixed in a manner to hold the energy isolating devices in a "safe" or "off" position. On December 17, 2019, employees were exposed to machine hazards associated with moving parts while cleaning rollers on the Pattyn line filler machine. The company failed to implement it energy control procedure for locking out a roller conveyor prior to cleaning off oil from the roller conveyor.
