29 CFR 1910.178(a)(4): Modifications and additions which could possibly affect capacity and safe operation were performed by the customer without manufacturers prior written approval. a) A Zone department - On or about 12/18/2019, and times prior to, the employer exposed employees to struck-by hazards in that the employer failed to receive written approval from the manufacturer of powered industrial truck for modification and additions that affected safe operation of powered industrial trucks when employees were operating Raymond pallet jacks that were modified by the employer by adding carts attachments to the pallet jacks.

29 CFR 1910.178(l)(3)(i)(G): Powered industrial truck operators did not receive training on attachment adaptation, weight of the attachment, and use limitations; a) A Zone department - On or about 12/18/2019, and at times prior, the employer exposed employees to struck-by hazards in that the employer failed to provide training for pallet jacks connected to the cart attachments to employees operating pallet jacks for loading and unloading boxes of the products when the cart attachments were added to the pallet jacks.
