29 CFR 1910.159(c)(10): The minimum vertical clearance of 18 inches between automatic sprinkler systems sprinklers and the material below was not maintained:  a)The employer is failing to protect employees from smoke inhalation and burn hazards associated with fires while working in the evidence vault. This was most recently documented on 12/19/2019 at 210 Walnut Street, Rm 707-B, Des Moines, IA 50309. A wooden rack system is installed in the center of the room and obstructs the sprinklers ability to overlap and pre-wet combustibles.    The Bureau of Alcohol, Tobacco, Firearms, and Explosives was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.159(c)(10), which was contained in OSHA inspection number 1057054 , citation number 2 item number 2 and was affirmed as a final order on 5/27/2015, with respect to a workplace located at 915 2nd Ave. Ste. 790, Seattle, WA 98174.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a) The employer is failing to protect employees from fire and electrical hazards while utilizing relocatable power taps (known as power strips). This was most recently documented on 12/19/2019 at the site location of 210 Walnut Street, Rm 707, Des Moines, IA, 50309. An employee had a relocatable power tap was plugged in to an extension cord (connected in series) to power a fan, phone charger, and radio.   b) The employer is failing to protect employees from fire and electrical hazards while utilizing relocatable power taps (known as power strips). This was most recently documented on 12/19/2019 at the site location of 210 Walnut Street, Rm 707, Des Moines, IA, 50309. An extension cord was plugged into a relocatable power tap (connected in series) to power a computer charging cord.   c) The employer is failing to protect employees from fire and electrical hazards while utilizing relocatable power taps (known as power strips). This was most recently documented on 12/19/2019 at the site location of 210 Walnut Street, Rm 707, Des Moines, IA, 50309. An electric space heater was plugged into a relocatable power tap.    The Bureau of Alcohol, Tobacco, Firearms, and Explosives was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.303(b)(2), which was contained in OSHA inspection number 1391300, citation number 1, item number 1 and was affirmed as a final order on 06/25/2019, with respect to a workplace located at 1 Poston Rd, Charleston, SC 29407.  The Bureau of Alcohol, Tobacco, Firearms, and Explosives was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.303(b)(2), which was contained in OSHA inspection number 1193159, citation number 1, item number 3 and was affirmed as a final order on 11/17/2016, with respect to a workplace located at 200 South Virginia St Ste 600 Reno, NV 89501.  The Bureau of Alcohol, Tobacco, Firearms, and Explosives was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.303(b)(2), which was contained in OSHA inspection number 1103280, citation number 2, item number 3 and was affirmed as a final order on 11/03/2015, with respect to a workplace located at 8965 S Eastern Ave Ste 200 Las Vegas 89123.
