29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a.  On or about December 19, 2019, at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three (3) feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a.  On or about December 19, 2019, at the above addressed jobsite, an employee performing residential roofing operations nine (9) feet above the ground, was not protected from falls, thereby exposing the employee to fall hazards.  b.  On or about December 19, 2019, at the above addressed jobsite, an employee performing residential roofing operations seventeen (17) feet eleven (11) inches above the ground, was not protected from falls, thereby exposing the employee to fall hazards.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a.  On or about December 19, 2019, at the above addressed jobsite, employees had been provided fall training, however, the employer failed to keep a written certification of record.  EKA Corp., dba EKA Construction Corp. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.503(b)(1), which was contained in OSHA inspection number 1412365, citation number 2, item number 1, and was affirmed as a final order on November 11, 2019, with respect to a workplace located at 11331 S. Belmont Dr. in Plainfield, IL.  EKA Corp., dba EKA Construction Corp. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.503(b)(1), which was contained in OSHA inspection number 1411432, citation number 2, item number 1, and was affirmed as a final order on November 11, 2019, with respect to a workplace located at 11323 Preakness Dr. in Plainfield, IL.
