29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance paragraph (b) or (c) of 29 CFR 1926.652: a) Jobsite - Employees were exposed to cave-in hazards in that the employer allowed the employees to work in a trench, approximately 19.5 feet long by 9 feet wide and 9 feet deep, without having a trench protection system in place. This condition was observed on or about December 19, 2019.

29 CFR 1926.651(b)(4): While the excavation was opened, underground installations were not protected, supported or removed as necessary to safeguard employees: a) Jobsite - The employer allowed employees to work in an excavation, approximately 19.5 feet long by 9 feet wide and 9 feet deep, where the underground water utilities were not supported thus exposing employees to hazards associated with water entering the excavation. Noted on or about 19 DEC 2019.
