29 CFR 1926.1060(b): The employer did not provide retraining as necessary so that each employee exposed to fall hazards maintained the understanding and knowledge acquired through initial training:(29 CFR 1926.1060(a)(1)(iii): On or about December 17, 2019, at a construction jobsite, an HVAC insulator used a ladder that was not extended three (3) feet above and/or secured to a rigid support to allow safe access and unintentional displacement of the ladder when ascending and descending. Employees were exposed to fall hazards.
