29 CFR 1910.23(e)(3)(ii): Mobile ladder stand platforms with a platform height of 4 to 10 feet (1.2 m to 3 m) did not have, in the platform area, handrails with a vertical height of at least 36 inches (91 cm) and midrails: (a) On or about December 19, 2019, employees in the basement level of the Extrusion Department used a mobile ladder stand platform with a platform deck height of approximately five (5) foot above the floor. Surrounding the platform was a two rail handrail/guardrail system with a top handrail/guard rail approximately 36 inches above the platform deck. On the platform deck a second step platform was attached giving the employee an extra foot of height to be able to see into the molten extrusion process. While standing on this top step the employee was enclosed by an approximately 24 inch high handrail/guard rail to protect the employee from a fall. This exposed employee(s) to a six (6) foot fall hazard to the ground below.

29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: (a) On or about December 19, 2019, an employee working in and near the extrusion department, used a 3M N95 tight fitting, face filtering mask type respirator. The employee was not clean shaven affecting the seal of the respirator, exposing the employee to inhalation of airborne lead dust while performing work tasks associated with lead materials.
