29 CFR 1910.147(c)(4)(ii)(B): The lockout tagout program procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: a) Detail Department. On or about January 24, 2020, and at times prior thereto, employees were exposed to amputation hazards while operating the HTC 10-foot hydraulic press brake, Model S-631, without specific lockout tagout procedures in place. b) Detail Department. On or about January 24, 2020, and at times prior thereto, employees were exposed to amputation hazards while operating the Steelweld 8-foot hydraulic shear, Model 10-D-8, without specific lockout tagout procedures in place. c) Detail Department. On or about January 24, 2020, and at times prior thereto, employees were exposed to amputation hazards while operating the Hyd-Mech horizontal bandsaw, Model S-23A, SN XA1204303FD, without specific lockout tagout procedures in place. d) Detail Department. On or about January 24, 2020, and at times prior thereto, employees were exposed to amputation hazards while operating the Hyd-Mech horizontal bandsaw, Model S-25A, SN 90290188, without specific lockout tagout procedures in place. e) Detail Department. On or about January 24, 2020, and at times prior thereto, employees were exposed to amputation hazards while operating the Hyd-Mech horizontal bandsaw, Model M20-A, SN TAB061S1121, without specific lockout tagout procedures in place. f) Detail Department. On or about January 24, 2020, and at times prior thereto, employees were exposed to a struck-by hazard, while preparing material behind the Steelweld Shear, Model 10-D-8, without a barrier or guard in place.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, was not guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle: a) In the Detail Department: On or about January 24, 2020, and at times prior thereto, employees were exposed to an amputation hazard, while bending steel on the HTC 10-foot press brake, Model S-631, as it did not have a guard at the point of operation. b) In the Detail Department: On or about January 24, 2020, and at times prior thereto, employees were exposed to an amputation hazard, while shearing steel on the Steelweld 8-foot shear, Model 10-D-8, which had a section of the guard missing.
