29 CFR 1910.147(d)(2): Machine or equipment shutdown. The machine or equipment shall be turned off or shut down using the procedures established for the machine or equipment. An orderly shutdown must be utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage. On or about December 13, 2019 failed to ensure that the F7 thermal forming machine was turned off or shut down as required. An employee was not protected from the hazardous energies and moving mechanical parts of the equipment when attempting to remove excess material from the output portion of the press. The equipment was not locked out or tagged out at the time and was in operation mode.
