29 CFR 1928.57(a)(6)(v): Lock out of electrical power was not conducted prior to performing maintenance or service on farmstead equipment: 1330 Newark Road, Toughkenamon, Pennsylvania - Employee exposed to unexpected startup of equipment in that the employer did not effectively lock out hazardous electrical energy to the ventilation blower prior to performing maintenance on equipment. Noted on or about 11 DEC 2019.

29 CFR 1928.57(a)(11): Where a moving machinery part presented a hazard during servicing or maintenance, the power source was not disconnected, and all machine movement was not stopped before servicing or maintenance was performed, except where the Employer could establish the following in 1928.57(a)(11)(i-iii): a) 1330 Newark Road, Toughkenamon, Pennsylvania - 1330 Newark Road, Toughkenamon, Pennsylvania - During maintenance of ventilation blower, the Employer did not ensure that the electrical power source was properly disconnected to prevent employee exposure to moving parts. An employee sustained a finger amputation when his finger was caught between the sheave of the pulley and the fan belt during maintenance of the ventilation blower. Noted on or about 11 DEC 2019.
