29 CFR 1910.28(b)(15): The employer did not ensure each employee on a walking-working surface 4 feet (1.2m) or more above a lower level is protected from falling: a) Putty Line; On or about December 9, 2019 and at times prior thereto the employer exposed employees to seven foot fall hazards in that employees were allowed to cross over a handrail and replace belts on the putty line raised conveyor without fall protection.

29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a) Putty Line; On or about December 9, 2019 and at times prior thereto the employer exposed employees to potential hazardous energies in that employees were allowed to replace belts on the putty line without shutting it down and locking it out in accordance with the company's Lockout program established procedures.
