On or about December 16, 2019, at the jobsite located at 308 West Walton Street, Warrenton, Missouri, the employer did not ensure employee(s) operating the Deshazo Bridge Crane (Model: 75005850; Serial Number: 10758; Capacity: 10,000 pounds) brought the hook over to the load in such a manner to prevent swinging.
