29 CFR 1926.453(b)(2)(iv): Employees did not always stand firmly on the floor of the basket, and sat or climbed on the edge of the basket or used planks, ladders, or other devices for a work position.    On or about November 15, 2019, and times prior thereto, employees climbed the mid and top rails of the basket of an aerial lift to gain access on and off a roof exposing employees to fall hazards of approximately 24 feet to the ground below.

29 CFR 1926.453(b)(2)(v): A body belt or harness was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated.    On or about November 15, 2019, and times prior thereto, employees operated aerial lifts without the use of a full body harnesses and lanyards attached to the basket exposing the employees to unprotected fall hazards and ejection hazards of approximately 24 feet to the ground below.
