29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: a) On or about December 27, 2019, the employer did not ensure that employees required to operate powered industrial trucks, including, but not limited to the Z-Nissan MCP1B2L20S Forklifts, S/Ns CP1B2980083, 404FCC2519593, CP1B29V0937, and CO1B29C0929 were re-evaluated at least once every three years. The last date employees received refresher training was March 8, 2016.
