29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  a) On or about 12/26/19 at a project located at 319 Connecticut Street, Buffalo, New York; employees were using a boom lift to access work areas while performing roofing work on the side of the building. The employees in the lift did not have harnesses with lanyards attached to the boom or basket.  NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) On or about 12/26/19 at a roof project located at 319 Connecticut Street, Buffalo, New York; employees were replacing the roof of a two story building. The roof had an 10 in 12 pitch and an eave height of approximately 20 feet. There was no fall protection in use.  NO ABATEMENT CERTIFICATION REQUIRED
