29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: Worksite located at Woods Crest Dr & Woods Chapel, Lee's Summit MO -- Employees were exposed to struck-by and crushed by hazards as the employer did not provide or ensure that a ladder or other means of egress was available for employees working in the trench. The foreman was working in the trench which was approximately 9' long X 12' wide X 5.8' deep. This documented on or about 12/31/2019.

29 CFR 1926.652(a)(1): Employees were not protected in an excavation from a cave-in by an adequate protective system designed in accordance with paragraph (b) and (c) of this section. Worksite located at Woods Crest Dr and Woods Chapel, Lee's Summit MO -- The employer did not provide or require the use of benching, sloping, shoring or other means to ensure that employees working in the excavation was protected from engulfment hazards. The trench was greater than 5 feet in depth. This was documented on or about 12/31/2019.
