In that, pre-entry air monitoring was not conducted, periodic air monitoring was not conducted, energy sources were not locked out, entry permits were not completed, procedures were not developed for summoning rescue services, rescue and emergency equipment was not used, and an authorized attendant was not present outside the confined spaces during entry.
