29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about September 17, 2019 the employer failed to ensure that the material kicker arm (s) of line 17 was effectively guarded to prevent employees from the hazardous area. b) On or about September 17, 2019 the employer failed to ensure that the material kicker arm (s) of line 18 was effectively guarded to prevent employees from the hazardous area. c) On or about September 17, 2019 the employer failed to ensure that the material kicker arm (s) of line 20 was effectively guarded to prevent employees from the hazardous area. d) On or about September 17, 2019 the employer failed to ensure that the material kicker arm (s) of line 21 was effectively guarded to prevent employees from the hazardous area. e) On or about January 9, 2019 the employer failed to ensure that effective machine guarding was in place on the pinch point of the area where the conveyor meets the end pulley on line 20. f) On or about January 9, 2019 the employer failed to ensure that effective machine guarding was in place on the pinch point of the area where the conveyor meets the end pulley on line 21.
