29 CFR 1926.153(o): When damage to LP-Gas systems from vehicular traffic was a possibility, precautions against such damage were not taken: A.) Frontier General Contracting, Inc., 3901 Pacific Avenue, Erie, PA 16506, west side of the structure, Grey LPG Tank: On or about December 27, 2019 and times prior thereto: Vehicles including, but not limited to, work trucks and work van were driving past an LPG tank. The LPG tank was not properly protected against potential vehicular damage. B.) Frontier General Contracting, Inc., 3901 Pacific Avenue, Erie, PA 16506, west side of the structure, White LPG Tank: On or about December 27, 2019 and times prior thereto: Vehicles including, but not limited to, work trucks and work van were driving past an LPG tank. The LPG tank was not properly protected against potential vehicular damage.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: A.) Frontier General Contracting, Inc., 3901 Pacific Avenue, Erie, PA 16506, west side of the structure, canopy roof, on or about December 27, 2019 and times prior thereto: Employees of the sub-contractor were performing roofing activities that included, but were not limited to, installing the cap on the peak of the roof structure. Employees were not utilizing means or methods of fall protection when exposed to an eave-to-ground height fall hazard of 11.7-feet, and a peak-to-ground height fall hazard of 18.2-feet. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

Frontier General Contracting, Inc., 3901 Pacific Avenue, Erie, PA 16506, south side of the structure, main roof, on or about December 27, 2019 and times prior thereto: Employees of the sub-contractor were performing roofing activities that included, but were not limited to, installing the cap on the peak of the roof structure. Employees were not utilizing means or methods of fall protection when exposed to an eave-to-ground height fall hazard of 11.7-feet, and a peak-to-ground height fall hazard of 23.3-feet. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
