29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolding was not fully planked or decked:  The employer is failing to ensure that each employee working on the working levels of a scaffold were protected from falling. This was most recently documented on or about January 2, 2020 at a residential construction site located at 10025 Crystal Water Bay, Lincoln, NE 68527. The employer had not fully planked all working levels of the scaffold employees worked.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:  The employer is failing to ensure that each employee working on the working a scaffold were protected from fall hazards. This was most recently documented on or about January 2, 2020 at a residential construction site located at 10025 Crystal Water Bay, Lincoln, NE 68527.  In numerous instances, the employer had not utilized mudsills and/or leg plates properly.

29 CFR 1926.451(g)(2): The employer did not have a competent person determine the feasibility and safety of providing fall protection for employees erecting or dismantling supported scaffolds:  The employer is failing provide a competent person exposing employees to fall hazards. This was most recently documented on or about January 2, 2020 at a residential construction site located at 10025 Crystal Water Bay, Lincoln, NE 68527. A competent person could have identified the multiple visible defects of the scaffold and corrected them before employees began working.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  The employer is failing to train all employees that were potentially exposed to fall hazards to enable each employee to recognize the hazards associated with the type of work. This was most recently documented on or about January 2, 2020 at a residential construction site located at 10025 Crystal Water Bay, Lincoln, NE 68527. This should include training on mitigating those hazards through demonstration of properly constructed scaffolding.
