29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: On or about December 30, 2019, and at times prior thereto, the employer did not get manufacturer's written approval to add fork lift extensions to the Toyota powered industrial truck, exposing employees to the hazard of being struck by the load.

29 CFR 1910.178(o)(1): 29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads shall be handled. Caution shall be exercised when handling off-center loads which cannot be centered. On or about December 30, 2019, and at times prior thereto, the employer did not ensure that the parking lot/security booths were stable while being unloaded from the semi-truck, exposing employees to a struck by hazard.
