29 CFR 1926.502(i)(3): All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees: A) Worksite: Employees undergoing rebar, framing and housekeeping work on the 9th floor were exposed to slip and trip hazards when walking across unsecured floor hole covers that measured approx. 12 inches by 6 inches. On or about 1/3/2020. b) Worksite: Employees undergoing rebar, framing and housekeeping work on the 11th floor were exposed to slip and trip hazards when walking across unsecured floor hole covers that measured approx. 12 inches by 6 inches. On or about 1/3/2020. Note: Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
